Health Screenings
- All VPK, Kindergarten, 1st and 3rd grade students are screened for vision, hearing (and height/ weight in some grades). A waiver is available if you have regular doctor appointments and would not want your child screened. Please return the following if you do not want your child screened.
- Health Screen Opt-Out Letter 2026-27
- Any student who has a specialized learning plan (504, IEP) will be screened at the start of their plan and when there are changes made to their plan.
- If you receive a referral, follow the instructions and return it to the clinic.
